TITLE
RESOLUTION AUTHORIZING THE MAYOR OF THE CITY OF NEW HAVEN TO APPLY FOR AND ACCEPT A PREVENTATIVE HEALTH AND HEALTH SERVICES GRANT AWARD OF $300,000 FOR THE PERIOD OF OCTOBER 1, 2024 TO September 30, 2028 FROM THE CONNECTICUT DEPARTMENT OF PUBLIC HEALTH AND TO EXECUTE, ACKNOWLEDGE, IMPLEMENT AND DELIVER ANY AND ALL DOCUMENTS AS MAY BE CONSIDERED NECESSARY OR APPROPRIATE WITH RESPECT THERETO.
BODY
WHEREAS, the New Haven Health Department recognizes the importance of providing New Haven’s residents with programs aimed at reducing unintentional injury; and,
WHEREAS, the City of New Haven will apply for funding from the Connecticut Department of Public Health; and upon receipt of such grant, the City will accept said grant in its entirety; and,
WHEREAS, the Connecticut Department of Public Health has provided the City of New Haven Health Department the opportunity to apply for funding under the Preventive Health and Health Services Grant for the provision of programs that prevent unintentional injury among New Haven residents; and
WHEREAS, the Connecticut Department of Public Health has changed the award amount for the City of New Haven from $500,000 to about $300,000; and,
WHEREAS, the Connecticut Department of Public Health has extended the award contract term from ending on September 30, 2024 to ending on September 30, 2028.
NOW, THEREFORE, BE IT RESOLVED by Board of Alders of the City of New Haven that the Mayor, on behalf of the City, is authorized to apply for and accept the grant mentioned herein from the Connecticut Department of Public Health.
BE IT FUTHER RESOLVED that the Mayor is authorized to execute said application for funding for a revised amount of about $300,000 and to execute any revisions, contracts, amendments or modifications as necessary.
